What is All-on-6 technology?
What is All-on-6 technology?
Dental implantation under the all-on-6 protocol involves installing six dental implants in the patient’s upper and/or lower jaw to restore lost teeth.
To replace the natural root of the tooth, we use one- or two-part implants with screw attachment or Multi-Tool abutments that provide reliable fixation in the jaw’s bone structures.
The implant placement scheme involves installing two units in the frontal zone and four more in place of canines and molars.
Features
All-on-6 dental implantation is a version of all-on-4 technology that uses four implants. In both cases, the lateral artificial roots of the teeth are located not vertically but at an angle, which allows the installation even with severe jawbone atrophy.
It minimizes the risk of nerve and sinus damage, as in the classical vertical implant placement.
Features of the All-on-6 implantation protocol:
- six dental implants are used;
- installation of implants in the lateral area at an angle;
- installation of the prosthesis immediately after implantation or a maximum of 24 hours after surgery.
Thanks to the placement of artificial supports in several zones, we achieve an even distribution of chewing load, minimize soft tissue injuries, and, accordingly, reduce the risk of implant rejection.
Placement of the prosthetic structure on 6 implants is one of the protocol types for one-stage dental implantation with instant load
Requirements for implantation
The use of all-on-six technology is required in the following cases:
- Full or partial adentia;
- severe mobility of teeth that requires their removal;
- Mild to moderate jaw bone resorption;
- atrophy of the alveolar appendage on the upper jaw
- the need for prompt restoration of the integrity of the dentition.
Installation of the All-on-6 denture is sometimes performed according to a pre-made surgical prototype.
Thanks to this, it is possible to install implants using a micro-invasive method and implant the body of the jaw bone as accurately as possible.
Stages of treatment
The need to install a fixed prosthesis on 6 implants is determined by the dentist after examining and diagnosing the patient’s oral cavity.
If there are no severe contraindications, then implantation using the All-on-6 method is performed in three stages.
Stage 1: preparing for surgery
Restoration of teeth using All-on-6 technology in one day requires careful planning of the operation and, in particular, accurate selection of dental implant placement areas.
To do this, we use computed tomography during diagnostics in dentistry, which allows seeing the structure of the jaw tissues as accurately as possible and selecting areas with a sufficient amount of bone mass.
After that, based on CT images, the dentist models a three-dimensional model of the jaws and chooses the optimal parameters of the implants. Next, he makes a treatment plan.
Stage 2: implantation
All all-on-6 implants are installed in one visit within 2-3 hours. To do this, the dentist makes a puncture in the gum, where he installs the implant.
The primary method used is patchwork, which involves exfoliating the gums and allows implanting all 6 implants in the deep layers of the bone.
Stage 3: prosthetics
The prosthetic structure is installed immediately after the implants or a few days after surgery. First, we use a lightweight acrylic prosthesis printed on a 3D printer.
It does not put excessive pressure on the implants. It is crucial to minimize injury to the soft and bone tissues of the jaw and ensure the fastest engraftment of implants.
After 3-6 months, we replace the temporary prosthesis with a permanent structure made of zirconium or other material of the patient’s choice.
Advantages
All-on-6 dental implantation:
- restoration of teeth in one session;
- possibility of implantation without bone grafting;
- high reliability of the prosthetic structure;
- low injury rate of surgery with minimal risk of complications;
- aesthetics of the prosthetic structure.
Types of jaw prosthetics with 6 implants
Implantation of the upper jaw with subsequent prosthetics is characterized by specific difficulties comparing to similar manipulations on the lower jaw due to some anatomical features.
The maxillary bone is looser and thinner in structure, and implants take longer to take root. The proximity of the maxillary sinuses also plays a role. Their topography has to be considered when installing implants.
To increase the implantation success, I recommend installing 6 supports on the upper jaw in the absence of teeth, while on the lower jaw, the limit is 4 implants.
Treatment tactics are selected individually for each patient, where the doctor considers all factors that may affect implantation results.
At first, the patient will wear a securely fixed temporary prosthesis made of acrylic. It has several advantages over the removable version:
- securely fixed;
- does not affect diction;
- looks beautiful and aesthetically pleasing.
The prosthesis is created at the stage of planning the procedure and fixed to the abutments. Sometimes on the same day, sometimes in a couple of days, when the puffiness disappears.
The service life of a high-quality prosthesis is several years, so some patients are in no hurry to consider permanent prosthetics.
At the patient’s choice, a permanent prosthesis is made of zirconium dioxin or cermet.
Zirconium is the best option, although it has a high cost. The material looks as natural as possible and has high strength, which allows it to be used both in the smile area and for prosthetics of chewing teeth.
When wearing it, there is no allergy to metal or the effect of electroplating. The reliability and biocompatibility of zirconium are very high, so if you are planning implantation, it is better to include this option in the estimate.
Dental cermets are not only a more budget-friendly choice, but they also have high strength and reliability. In the creation of such prostheses, we:
- achieve maximum naturalness;
- make sure that the metal does not show through from the inside;
- combine materials to enhance aesthetics.
What is the price of implantation and prosthetics “All-on-6”?
The cost of the procedure varies depending on the complexity degree of each specific case and the materials used.
Despite many implants offered on the market, I work only with high-quality and well-proven materials. Their cost fully corresponds to the quality.
Most of the designs are unique patented technologies from Israel and Switzerland. Their shape and coating best contribute to conjoining with the bone.
Therefore, in 99.5% of cases, implantation is successful not only in terms of efficiency but also in terms of the duration of trouble-free operation.
The service life of branded implants is decades. With proper care, they will last a person’s entire life. They don’t have to be changed, and this is a great investment.
As for the second component of the procedure – materials for making a prosthesis, it also all depends on what material the patient chooses.
